I hereby request a prescription for medication so I can purchase this product from an internet pharmacy or catalog vendor not associated with Seven Hills Veterinary Hospital. By signing this waiver, I acknowledge that I have been properly informed of the risks involved in doing so and accept any and all responsibility, financial or otherwise, that may occur from this decision. I understand that Seven Hills Veterinary Hospital and its associated staff will be held harmless from the use of products and/or prescriptions purchased from sources outside of the hospital’s monitoring and control.

  1. There is the possibility that the prescription drugs received from these vendors may be counterfeit and may not have been approved by the FDA, and pesticides may not have been approved by the EPA.   2. You may not have the prescription drugs available soon enough to start your pet’s medications at the optimal time to provide relief for their condition.
  3. Manufacturer rebates that would ordinarily be available for products purchased from us are not available.
  4. Manufacturers' warranties or guarantees for these products are not valid with internet or catalog prescriptions. This means if your pet’s condition is not effectively treated with the product, the product manufacturers may not stand behind their products or product liability procedures.
